Please be aware of our 48-hour cancellation or rescheduling policy: Due to appointment demand, any appointment cancelled or rescheduled with less than a 48-hour notice for any reason will be charged a fee of $50.00 added to your next scheduled visit.

Parking & Entrance

Park in the rear of the building. The entrance is accessible on side of the building looking from the parking lot where you will see three concrete stairs and a handicapped accessible ramp. Enter through the white door where you will see a Circle of Health sign on the door.

The waiting room is to the right after entering the building.
